Parents may utilize this form to restrict titles their child may check out in the school library. Please complete the form and return it to your child's teacher or librarian. This form is only required when a parent is seeking to restrict access to materials in the media center.
POLICIES & PROCEDURES
K-5 students will visit the Media Center regularly as part of their resource rotations.
Middle School students … TBD
Open check out – students may visit the Media Center independently during the school day with their teacher’s permission and a pass.
Check Out Limits:
Grades K – 1 = 1 book at a time
Grades 2 – 4 = 2 books at a time
Grades 5 – 7 = 3 books at a time
Note: Only 1 SSYRA book and only 1 book from the same series may be checked out at the same time.
Due Dates:
Books are checked out to students for 3 weeks in Destiny. K-5 students are expected to have their books with them on their Media resource day. We are happy to take early returns to make finished books available to other students. Students may renew their books for another 3 weeks as long as there are no holds on the book.
Students with overdue materials or fees need to clear their account before being allowed to check out new materials.
Holds:
Students may place a hold for a book that is currently unavailable in person or at home via Destiny. Classroom teachers will be notified when student holds are ready to pick up. We will hold books for 3 school days before the book goes back on the shelf or to the next patron on the waitlist.
Book Care:
Please encourage your children to take care of the library books they borrow by keeping them safe in a protective bag within their backpack when they are not being read and returning them on time and in good condition, so they are available for others to read. We appreciate our partnership with our families as we love seeing our students read!
Lost & Damaged Materials:
Students and their parents are responsible for paying the replacement cost for lost or damaged books. If a lost book is found and returned to the Media Center by the end of the same school calendar year in good condition, monies will be refunded. If an item is returned after a calendar school year has closed, the item will be considered a donation to the HCA Media Center.
Fee Payments:
The following payment methods are accepted for lost and damaged books: Cash (exact), Check (made payable to HCA), and online using this link to SchoolPay (coming soon). Please note > if you owe book fees to another SJCSD school (not HCA), those fees must be paid to that school. You may send in cash (exact amount) or check (made out to the appropriate school) and we will put it in our inter-school mail to that school’s media specialist to process your payment.
If your student receives a printed fee notice from the Media Center or you receive an email notice via Destiny, please send payment to school in a sealed envelope with your student’s full name, teacher’s name, and the book(s) you are paying for.
When paying online using School Pay, be sure to email your payment receipt to one of our media staff members so we can mark your fee paid, thereby clearing your student’s account so they can return to full circulation privileges.
