Health Screening Opt-Out Form
In compliance with Florida Statute 381.0056, students will receive health screenings for vision, hearing, height/weight, and scoliosis (grade levels vary). These screenings help identify potential health concerns that could impact learning. Parents who do not wish for their child to participate may complete and return the Health Screening Opt-Out Form to the school nurse by October 15, 2025.

About Our Nurse
Nurse Jana is so excited to be Hallowes Cove Academy’s school nurse! She was born and raised in Jacksonville Beach and now lives in Rivertown with her husband. She has two wonderful daughters, now 29 and 31, and she couldn’t be prouder of them.
Nurse Jana’s husband works 12-hour shifts that flip from days to nights every four weeks, and that unique schedule is actually what first led her into school nursing. She spent 12 years with Duval County Schools before exploring other areas of nursing after her children had graduated. However, she found herself missing the energy and connections that come with working in a school.
Nurse Jana is honored and happy to be back in school nursing here at HCA. Caring for students and supporting families is truly where her heart belongs!
